Who we are
April Brown, LPN
Co-Founder | Patient Advocate
April Brown is a compassionate and highly experienced Licensed Practical Nurse with more than 15 years of dedication to guiding patients and families through the often overwhelming healthcare landscape. With a background spanning physician offices, hospitals, and hospice care, she brings a well-rounded, real-world understanding of both the clinical and emotional aspects of patient care.
For the past six years, April has served as a Care Coordinator Nurse, becoming a trusted partner for families navigating complex and deeply personal healthcare decisions. She has extensive experience supporting transitions into assisted living facilities (ALFs), skilled nursing facilities (SNFs), palliative care, and hospice services—ensuring each step is approached with clarity, dignity, and thoughtful planning.
April’s passion for caring for others is deeply rooted in her own life experiences. Her close bond with her grandparents, especially her great-grandfather, profoundly shaped her calling. As a child, she witnessed the power of compassionate care when a hospice nurse supported her great-grandfather through his cancer journey. That moment left a lasting impression and inspired her path; by the age of 10, April knew she was called to serve others through nursing, with a special heart for the senior population.
Today, that same love for family continues to guide her work. April believes caring for others is not just a profession—it is her purpose. She is especially passionate about advocating for individuals living with dementia and Alzheimer’s disease, striving to preserve dignity, promote comfort, and ensure every patient receives personalized, respectful care.
As a lifelong Indiana resident, April brings a deep sense of community, empathy, and commitment to everything she does. Through Guided Path Patient Advocacy, she blends clinical expertise with genuine compassion, offering families not only guidance but peace of mind. Her mission is simple yet powerful: to ensure that every patient and loved one feels informed, supported, and never alone on their healthcare journey.
